The PTPN Outcomes Program helps you and your therapist work together toward better health, by collecting information about:
- how your injury or illness limits your daily functions.
- how your ability to function improves with treatment.
On your first and last visits, you will be asked to take a short survey about how your condition affects your daily life. The survey will cover such topics as:
- your health background.
- the area affected by your condition.
- your personal health status, including things like age, fitness level and health history.

When your therapist has more information on how your condition affects your daily activities,
- He or she will be better able to help you heal. Your survey responses will help your therapist to create a specialized program for your individual needs.
- You play an active part in your rehabilitation, giving you the chance to provide us with more information about your health and about how satisfied you are with your treatment.
- Your therapist’s team will know how effective their office is in getting patients healthy. Comparing each office’s data to a national database helps all PTPN members continue to improve their practices and deliver better service to all our patients.
