Our network, which is made up of over 135 independent rehabilitation clinics, is contracted with most of the area’s major health plans, as well as workers’ compensation carriers, TPAs and employer groups.

Meet the Arizona management team
Dawn Ripa, Executive Director
Dawn Ripa has played a key role in the development of PTPN since 1997, expanding the network from 25 clinics in 1997 to 138 clinics today. As Executive Director, she is responsible for the day‐to‐day operations of the region, including payer and provider contracting, marketing, provider relations, claims processing, overseeing of the Quality Assurance and Credentialing programs, and budgeting and analysis of overall network performance.
Lee Hall, Administrative Assistant
Lee Hall has been the Administrative Assistant and Referral Account Coordinator for PTPN Arizona since January 2005. Prior to coming to work for Arizona, he was with PTPN national in California for six years as a Provider Services Representative. Lee works closely with member clinics regarding credentialing and provider relation issues, as well as scheduling patients.
Veronica Lopez, Referral Coordinator
Veronica has been the Referral Coordinator for Arizona since March 2008. She coordinates therapy appointments for workers’ compensation patients who are referred through physicians, employers, claims adjusters and nurse case managers. She is also responsible for obtaining all authorization for therapy treatments.
Hazel Valentin, Provider Services Representative
Hazel has been working in the Arizona office since March 2008. She is responsible for all recredentialing site surveys and paperwork for member clinics.
Jerel Kaufax, Claims & Data Entry
Jerel is responsible for the data entry of claims from member clinics for all PTPN contracts. He has been with PTPN since 2008.
Quality Assurance and Utilization Review
Kelly Medlyn, PT, is the independent Quality Assurance and Utilization Reviewer for Arizona. She is responsible for all PTPN provider documentation and chart reviews. She also reviews and monitors utilization of therapy services for PTPN worker’s compensation clients.

Our Arizona office offers centralized scheduling for our workers’ compensation clients. Scheduling is done out of the Phoenix based office, affording easy access to over 135 PTPN providers statewide.
